Annexation and Census Forms
The forms in this section are provided for your convenience. OFM is happy to furnish a supply of any of these forms on request. Please contact Diana Brunink at (360) 902-0597. The Forecasting Division can be reached at (360) 902-0599 or e-mail ofm.forecasting@ofm.wa.gov. Whenever possible, please call in advance and request a supply of Annexation Certificates. These Certificates are provided on colored three-part (no carbon necessary) paper that helps to facilitate processing.
- Annexation Certificate - Please contact Forecasting to request the actual three color triplicate form that only requires signature on top form (no carbon needed). The three-part paper facilitates processing.
