The Washington State Office of Financial Management (OFM) maintains a central vendor file for Washington State agencies to use for processing vendor payments. This allows you, as a vendor, to receive payments from all participating state agencies by direct deposit, the State's preferred method of payment.

Instructions for completing these forms:
- Tab through the fields and type in your information. Detailed instructions for each field can be found on page two of each form.
- Print and sign both forms.
- Mail forms to the address at the bottom of page one of the registration form.
- For your records, you may save these forms to your computer as word documents by selecting "File" on your toolbar, and then selecting the "Save As..." option.
- These forms are secure, and do not transmit or store any of your information electronically.
